The Healthy Minnesota Partnership adopted Minnesota's most recent statewide health improvement framework on February 11, 2025. It includes three health priorities that the Partnership will focus on between 2025 to 2029:

Equitable access and care
Each health priority includes objectives and strategies that the Partnership will work on between 2025 to 2029. It is a dynamic and living plan that can evolve during implementation as progress is made, new relationships are built, new options emerge, or other changes occur.
Collaborative process
The statewide health improvement framework (SHIF) is a multi-year action plan to address prioritized issues in response to the Minnesota Statewide Health Assessment. The Partnership leads the collaborative planning and implementation process that includes many members and partners, a steering committee, community engagement, health priority workgroups, and ongoing consultation with health equity partners.
Purpose and uses
